About Us
In 1971, as home health agencies were beginning to emerge, Cozad Community Hospital leadership had the foresight to start a home care agency in Cozad. The first patients were seen in August of 1971. One RN and one home health aide comprised the entire home health staff. The Medicare & Medicaid certified Agency has since come to be known as Central Plains Home Health, covering 5 counties, employing 16 individuals who make over 7000 home visits each year.
In 1986, hospice services were added, providing care to the terminally ill. Central Plains Hospice began as a volunteer organization, until 1995 when it became certified by Medicare.
As a patient of Central Plains Home Health & Hospice, you can be sure that you are involved with an Agency, through its longevity and history, has been proven "tried and true."
